How do you handle getting Cat 5E (or I guess Cat 6) from your servers to the core switch? Just straight cable runs in the ceiling/under the floor? Patch panels? Or something else? Basically I want to ...

Just ignore the colors and use the 586A way. All what the patch panel is, is a passive device that all your cable comes to, and is then packed down to your switch rather then directly to you switch.
